Ankle Impingement

Ankle impingement describes pain caused by irritated soft tissue or bone at the front, back or side of the ankle, usually at the end of a particular movement. Anterior pain is often worse with deep dorsiflexion, while posterior impingement is aggravated by plantarflexion in activities such as kicking, dance and gymnastics. Previous ankle sprains and repetitive loading are common contributors. Assessment identifies the exact compartment involved and looks for instability, an osteochondral lesion or tendon disorder. Weight-bearing X-rays are often useful, with ultrasound, MRI or CT selected according to the suspected structure. Treatment begins with load adjustment, restoration of motion and strength, and sport-specific rehabilitation; a targeted injection or arthroscopic opinion is considered for persistent, well-defined structural symptoms.
